Breast lift: Procedure & realistic results
Scar techniques, areola correction, and what before-and-after photos realistically show – explained honestly by the FORUM Klinik.

After pregnancies, weight loss or simply with age, the breast loses its elasticity – it sags and the nipple points downwards. A breast lift (medically known as mastopexy) lifts and reshapes the breast without necessarily reducing its size. This guide explains the procedure, the different scar techniques, and above all, what is realistically possible and what a "before and after" really shows.
The honest part is important to us: a breast lift provides a firmer, more harmonious shape in natural proportions – but it leaves scars, and the result matures over months. Anyone who takes a realistic approach from the start will be more satisfied in the end. At the FORUM Klinik in Cologne, we advise you in exactly this way: objectively, in peace and quiet, and without promising you anything that cannot be sustained.
In Brief
The breast lift lifts and shapes the breast and places the nipple and areola in a harmonious position; if necessary, the areola is also reduced in size at the same time.
It can be performed alone (pure lift), combined with a reduction, or – in the case of volume loss – with autologous fat or an implant.
Depending on the severity, different scar techniques are used (periareolar, vertical, T-anchor).
Key data: General anesthesia, outpatient or one night, support bra for 6 weeks, downtime 10–14 days; the result matures over around 12 months.
For legal reasons, we do not show before-and-after pictures online – only during a personal consultation and with reference to your findings.
What is a breast lift?
During a breast lift, excess skin is removed, the breast tissue is reshaped into a firmer, higher position and the nipple-areola complex is moved to the appropriate place. The goal is to tighten and reshape the breast – with natural proportions instead of an artificial look. If the shape is the main priority rather than the volume, a pure lift (mastopexy) is sufficient. If the size or weight is also a concern, the lift is combined with a reduction. If, on the other hand, there is a lack of volume in the upper breast area after sagging, the lift can be combined with reconstruction using autologous fat or an implant. Which approach is suitable depends on the skin quality, volume and your goals.
When is a breast lift useful?
A good time is when
the breast has sagged significantly (ptosis) after pregnancy, breastfeeding, weight loss or due to age,
the nipple points downwards or lies below the inframammary fold,
there is excess skin and you want a firmer, naturally proportioned shape,
you are willing to consistently adhere to wearing the support bra and follow-up care.
To be honest: if family planning has not yet been completed, a subsequent pregnancy can change the result again – it is often wise to plan for this. And if there is actually only a lack of volume without significant excess skin, a lift is not always the right approach. We will clarify this in advance.
The breast lift procedure – step by step
Consultation. We discuss goals, symptoms and concerns, examine anatomy, skin quality and proportions, and explain the course of the scars, the realistically achievable shape and the healing process – transparently and in a relaxed setting.
Preparation. Photo documentation, measurements, if necessary imaging in accordance with guidelines, surgical consent and trying on the support bra. In addition, advice on medication, nicotine cessation, sleeping positions and everyday life after the operation.
The surgery. Under general anesthesia, we remove excess skin and, if necessary, glandular and fatty tissue, shape a firmer breast, move the nipple and areola into a harmonious position and close the skin with low tension. If necessary, we add liposuction at the edges. The procedure takes about 120–240 minutes and is performed as an outpatient or with an overnight stay.
Initial follow-up care. Pain management, wound checks, wearing the support bra 24/7, plus scar care according to instructions and step-by-step activation. The check-up appointments are firmly scheduled.
Scar techniques – which scar for which severity?
A breast lift leaves scars – how much depends on the amount of excess skin. We choose the technique so that only as much is cut as necessary:
Technique | When | Scar Pattern |
Periareolar | mild sagging | scar around the areola |
Vertical (standard) | moderate sagging | around the areola + vertical to the inframammary fold |
Vertical + short T / T-anchor | pronounced sagging shape | additionally in the inframammary fold |
To put it realistically: the more excess skin there is, the more scars there will be. We place the incisions as inconspicuously as possible, and the scars fade significantly over about twelve months – they do not disappear completely. This openness is part of our professional planning.
Nipple and areola: repositioning and reduction
A central part of the breast lift is the repositioning of the nipple. If the areola has stretched or enlarged due to sagging, it can be reduced in size and shaped during the same procedure (areola correction). This is exactly what many people who research "nipple reduction" are looking for: a harmonious, proportioned areola at the correct height.
Important and honest: the sensitivity of the nipple and areola often changes temporarily after the procedure and usually returns to normal over the following months; in rare cases, changes remain permanent. Breastfeeding capacity may also be limited depending on the initial findings and technique. We will discuss both openly before making a decision.
Realistic results – and what "before and after" really shows
Many people search for "breast lift before and after" to see what is possible – a understandable wish. Therefore, here is the honest evaluation.
What is realistically achievable: a firmer, higher breast in natural proportions, a harmonious nipple position and – in the case of a large, heavy breast – noticeable relief. What before-and-after does not show: your individual healing, your skin quality and time. The result matures over about twelve months, scars need this time to fade, and gravity, aging, weight fluctuations or a subsequent pregnancy continue to have an effect – a lift does not "freeze" the breast in time.
Why we do not show before-and-after pictures online: The German Therapeutic Products Advertising Act (§ 11 HWG) does not allow this. We are only permitted to use real images with a medical indication and appropriate notice – and we discuss them during a personal consultation, in relation to your anatomy. This is the more honest approach anyway: pictures on the web often show ideal individual cases with a different starting situation, different anatomy and favorable lighting. A before-and-after only becomes meaningful when the pose, light and case selection are honest – and when it matches your findings. That is exactly what we provide in the consultation for a breast lift in Cologne.
Healing and follow-up care
After about a week, swelling and tension are usually decreasing; during this time, you must wear the support bra consistently and take it easy. After a month, everyday life is usually back to normal, light sports are possible after clearance – full activity is usually possible after 6–8 weeks. After around six months, the shape and scars look much calmer and sensitivity stabilizes; after about twelve months, the result is considered mature and the scar healing is complete. Cessation of smoking and consistently wearing the support bra significantly support the process.
Risks and Safety
Like any surgical procedure, a breast lift carries risks. In the first few days, swelling, bruising, a feeling of tightness and temporary changes in sensitivity are normal. Wound healing disorders occur more frequently if the skin is pre-damaged or if nicotine is involved; a tissue-sparing technique, tested hygiene processes and close follow-up care help prevent this. Specific scarring abnormalities, asymmetries, palpable fatty tissue hardening or fluid accumulation (seromas) can occur. In addition, there are the general risks of surgery and anesthesia. We will discuss your individual risk profile and warning signs in detail – and the rule is: it is better to report something too often than not enough.
Frequently Asked Questions (FAQ)
Is a reduction always performed during a lift? No. A pure lift is possible if the shape is the main concern. If there is too much volume, it is combined with a reduction; if there is too little volume, it can be built up with autologous fat or an implant.
What scars remain after a breast lift? Depending on the severity, periareolar, vertical or as a T-anchor. The scars fade over about twelve months, but remain visible.
Can the areola be reduced in size? Yes. A stretched or enlarged areola can be reduced in size and shaped as part of the lift.
Can I still breastfeed after a breast lift? The ability to breastfeed may be limited depending on the technique and initial findings. We will discuss this before making a decision.
Does the sensitivity of the nipple change? Often temporarily; it usually normalizes over the following months, in rare cases changes remain permanent.
How long does the result of a breast lift last? Usually for a long time. However, gravity, aging, weight fluctuations and a subsequent pregnancy continue to have an effect – a stable weight helps to maintain the result.
Why don't you show before-and-after pictures online? The Therapeutic Products Advertising Act prohibits this. We discuss real recordings with a medical reference in a personal consultation – matching your anatomy.
When will I be able to work and play sports again? Office work is often possible early, the downtime is usually 10–14 days. Light exercise is common after 3–4 weeks, full activity after 6–8 weeks.
How much does a breast lift cost? That depends on the findings and technique. A binding sum will be given to you in a personal cost estimate after the consultation.
Conclusion
A breast lift shapes the breast to make it firmer, more harmonious and better proportioned – with honestly stated limitations: scars, individual healing and a result that matures over months. "Before-and-after" is less an image on the web than a realistic assessment of your findings.
