Which implant size is right for me? Round or anatomical? And how long do breast implants really last? The honest guide from Cologne.

“What size should it be?” – this question sounds like the easiest of the entire breast augmentation process. In truth, it is the most important and most frequently answered incorrectly. This is because the correct answer is not found in a catalog or in a cup size, but in your body: your ribcage, tissue, skin, and proportions help decide what will look beautiful and wear well in the long term.
This guide explains how a reputable size selection process works, what lies behind “round or anatomical”, how safe modern implants are – and how long they actually last.
The question of size: Why “as large as possible” is almost always the wrong answer
An implant is not an isolated decision – it becomes part of your body and must suit it. Four factors limit and guide the choice:
The ribcage base. The width of your natural breast base dictates which implant diameter will look harmonious in the first place. An implant that is too wide looks unnatural and can spill over at the sides.
Skin and tissue. Your tissue must be able to support and cover the implant in the long term. Too much weight stretches the skin and ligaments – today's result becomes the day after tomorrow's problem.
Your proportions. Shoulders, waist, height. The goal is a breast that looks as if it has always been yours.
Your life. Sports, profession, family planning – this honestly belongs in the consultation.
In practice, this means: not “ordering cup size C”, but finding the volume that fulfills your desired image and your anatomy together with the surgeon on the basis of measurements and trial implants. Milliliters instead of cup size – because cup sizes are anything but standardized depending on the manufacturer. If the skin and tissue have already yielded, the planning should also include the question of whether a breast lift should be part of the procedure.
Round or anatomical? The question of shape honestly explained
Round implants give the upper breast pole more fullness – the emphasized cleavage. They can also rotate in the implant pocket without being noticed. Anatomical, i.e., tear-shaped implants replicate the natural shape of the breast: flatter at the top, fuller at the bottom. They look particularly natural, but require precise, stable placement.
The honest truth: There is no “better”, only a “more suitable” – depending on the initial findings, tissue, and desired outcome. In slim patients with little of their own tissue, the implant position – under or over the chest muscle – also plays at least as big a role in naturalness as the shape.
How safe are modern breast implants?
Breast implants are among the most intensively studied medical devices of all – also because the industry has learned from its scandals: The consequence of low-grade industrial silicone was stricter testing, complete batch documentation, and a mandatory implant register.
Modern brand implants use cohesive silicone gel, which retains its shape and does not leak even if the shell is damaged, as well as multi-layered shells. What matters for you: Brand implants with CE certification and manufacturer warranty, a documented implant passport – and a surgeon who is transparent about the manufacturer and type.
How long do breast implants really last?
The honest answer: a long time, but not guaranteed for life. Modern implants do not have an expiration date and do not need to be routinely replaced after ten years – that is a persistent myth. As long as the check-ups show no abnormalities and you remain symptom-free, an implant can stay in place for many years to decades.
Reasons for a later replacement can be: a capsular contracture, i.e., the hardening of the natural tissue capsule around the implant, shell fatigue after many years – or simply the desire for a different size if the body has changed. This includes regular check-ups with palpation findings and, if necessary, ultrasound. They are the price of peace of mind, and it is low.
Size, shape, incision – how everything is decided together
The three decisions are interrelated: size and shape help determine which incision path makes sense – and vice versa. How long do breast implants last?
Breast implants are not medical devices that last a lifetime. Regular medical check-ups are therefore important even if you have no symptoms. As a guide, a lifespan of about 10 to 15 years is often specified. After this period, an implant change may become sensible or necessary, but it is not automatically required after a certain number of years. The crucial factors are the condition of the implants, the examination results, as well as possible changes or symptoms. Reasons for a replacement can be, for example, a rupture, capsular contracture, changes in shape or position, as well as a changed aesthetic desire.
Are round or anatomical implants better?
Neither of them is better: Round implants emphasize the upper cleavage and are insensitive to rotation; anatomical ones replicate the natural shape of the breast. What fits is decided by the initial findings, tissue, and desired outcome.
Do you notice if an implant is broken?

Yes – inform the practice team about your implants beforehand. There are special imaging techniques; additionally, ultrasound or MRI are often used for implant patients.
