Methods & Variants
Transconjunctival Technique (inside):
Access from the conjunctiva – no external scar. Suitable when fat pads are primarily a concern and the skin quality is good. The fat is reduced or repositioned (e.g., to the tear trough) for a soft contour.Transcutaneous/Subciliary Technique (Skin-Muscle Flap):
Fine incision under the eyelashes; advisable in cases of skin excess and/or muscle laxity. A combination of skin smoothing, fat modeling, and possibly lateral support maneuver (canthopexy/plasty) for stability of the eyelid margin.Fine Corrections & Additions:
Fat repositioning to brighten the tear trough, skin pinch for minimal skin folds, autologous fat/hyaluron (staggered over time) for volume refinement, laser/peels for fine wrinkles (planned separately).

Process – Step by Step
Consultation
We discuss wishes and concerns, examine eyelid tension, skin, fat distribution, and the tear trough, clarify possibilities & limits (e.g. pigmentation issues) and explain the healing process – calmly and transparently.
Preparation
Photo documentation, precise marking while seated, clarification, information on blood thinners/medication, cooling, eye drops/ointments, sleeping position (head elevated) and organization of the first few days.
Treatment/Operation
In local anesthesia ± sedation (or general anesthesia), depending on the technique, the inner access or the fine skin incision under the eyelashes is performed. Fat pads are reduced/repositioned, skin and possibly muscle are carefully smoothed; if necessary, lateral support thread (canthopexy). Careful, low-tension closure; cooling and final check.
First Aftercare
Targeted cooling intervals, drops/ointment as planned, rest without rubbing, sun protection. Suture removal (if skin incision) after about 5-7 days. Make-up/contact lenses after release. Follow-up visits are firmly scheduled.
Healing process
After one week, swelling and bruising usually decrease significantly; stitches (if present) are removed.
After two weeks, most traces can be well concealed; the appearance already looks fresher.
After one to three months, contour and scar settle down; fine residual swellings subside.
After six to twelve months, the final result becomes evident.

Risks & Safety
As with any procedure, swelling, bruising, and a feeling of tension, feelings of dryness or tearing may temporarily occur. Specific to the lower eyelid are conjunctival irritation/chemosis, asymmetries, under- or overcorrection (hollow effect), scar abnormalities, and rarely lid margin changes (e.g., 'scleral show', ectropion). We reduce risks through careful indication assessment, examination of lid tension, tissue-preserving technique, if necessary, canthopexy for lateral stabilization, and a clear aftercare (cooling, head elevation, avoiding rubbing, sun protection).
Important: Quitting smoking, avoiding blood thinners after consultation, and consistent care support a calm healing process. Please report any warning signs (sudden increasing pain, unilateral severe swelling/redness, vision problems) immediately – you have a clear emergency protocol.
